Your Online Record

Please note - anywhere below which mentions coming into the surgery is not the case during the current COVID-19 situation.

If you would like to register for online access please click here for more information.

If you would like to apply for proxy access or have a subject access request you can print and complete the form and either send it in the post or drop off at the surgery via the letter box at our Wakefield Road entrance.

Detailed Coded Medical Record

We now offer our patient's online access to their Detailed Coded Medical Record. If you already have access to online services you can make the request when logged in. If you would like to apply for this access which comes as part of the appointment and prescription on line service please call at the reception desk. If we already have your photograph attached to your patient record then providing we can verify a match we require no further ID. If we don’t have your photograph attached to your patient record please bring with you to Reception a copy of your photographic ID (e.g. driver's licence, ID card, passport) and proof of address. You will be registered for access to your records when staff are happy that you match the demographic details held on the medical record. We will take a photograph of you at Reception to add to your patient record. Access to online services is managed with the use of passwords which we strongly recommend you keep safe and do not share with anyone else.

It is not currently possible to provide you with access to your full electronic medical record.

Online Services Proxy Access

This can be granted for those who need to act on behalf of a patient. This may be a parent who requires access for a child (only granted for children up to the age of 14) or a patient with or without capacity who requires someone to act for them (form to be completed). If you think this may be of interest to you leave your details at Reception and we will contact you back. Please note - Proxy Access to Online Services for children is automatically withdrawn at the age of 14. At the age of 16 patients are able to have their own access to Online Services. Proxy access can be requested for a child after the age of 14 by completing and submitting a new request. The child will be invited to attend an appointment with a clinician where the implications of this will be discussed and competency assessed. The clinician will decide the appropriateness of the access request and, if approving proxy access, if any further review is required prior to the age of 16.

Subject Access Requests

If you would like to request information from your medical record or for a child you have responsibility that is not available/viewable on the online record then please complete the Subject Access Request form and return to the surgery via post or in person FAO the Practice Manager. Access is also available through your Apple and Android devices. The apps can be downloaded from iTunes or Google Play. (Please note that these are very much cut-down versions of the full online system and will NOT give you as much access as if you view using a web browser.)
