New Patient RegistrationReg form image

We are happy to accept new patients to our practice list who live within our practice area. If you would like to register please complete the registration forms here.

All new applicants wishing to join the list aged 14 years and over are invited to have a health check with the Practice Nurse to discuss service requirements. The reception staff will make arrangements for this appointment.

Children of 13 years of age or under will be registered as their parent/guardian places their application. Although patients are registered with the “Practice” and not an individual doctor they can request a preferred doctor when registering. When registering, you will be asked if you have a preference for a particular doctor who might then become known as your usual doctor.

All our new patients are invited to attend a consultation to discuss their health needs. We would encourage all our new patients to arrange this at their earliest convenience.

All Our Patients Have A Named Accountable GP

All GP Practices have been instructed by the current government to make sure their patients are aware that they have a Named Accountable GP.

When you register at The Waterloo Practice you become a patient of the practice, registered with the practice and not an individual doctor. This means that you can see the GP of your choice and should that GP ever leave the practice you are not left without a doctor.

However, we have encouraged our patients to exercise continuity of care by identifying a "Usual Doctor" with whom they can build a relationship so that the doctor becomes familiar with their needs and is able to offer the most effective care. The usual doctor takes the lead responsibility for the co-ordination of services to the patient in exactly the way the Accountable GP must. If you wish to enquire as to who your usual / accountable GP is please enquire when you next have contact with the surgery.
